What is Vestibular Migraine?

Vestibular migraine is characterized by dizziness, imbalance, and other vestibular symptoms occurring during or outside migraine attacks. It commonly affects individuals with a history of migraines and involves the vestibular system, which is connected to the inner ear and brain.

Symptoms of Vestibular Migraine

  • Dizziness (sensation of spinning or swaying)
  • Imbalance or lightheadedness
  • Nausea and vomiting
  • Migraine-type headache (not always present)
  • Sensitivity to light and sound
  • Visual disturbances (blurred vision, flashing lights)

How is Vestibular Migraine Different from Other Types of Vertigo?

The key distinction of vestibular migraine is the association of dizziness episodes with migraine-like symptoms (e.g., light sensitivity) or migraine-type headaches. Other types of vertigo, such as BPPV (Benign Paroxysmal Positional Vertigo), are typically triggered by head movements or positional changes and do not include migraine-related symptoms.

Causes of Vestibular Migraine

Although the exact cause is unknown, several factors are thought to contribute:

  • Migraine sensitivity (genetic predisposition)
  • Dysregulation of the vestibular system in the brain
  • Hormonal changes
  • Triggers like stress, irregular sleep, and poor diet

How is Vestibular Migraine Diagnosed?

Diagnosis is based on clinical evaluation and the patient’s symptom history. The relationship between dizziness, migraine history, and vestibular symptoms is key. While there is no specific laboratory test for vestibular migraine, certain tests may be performed to rule out other conditions.

Treatment for Vestibular Migraine

1. Lifestyle Modifications

  • Maintain regular sleep and meal schedules.
  • Avoid migraine triggers (e.g., caffeine, alcohol, chocolate).
  • Practice stress management techniques.

2. Medications

  • Preventive migraine medications (e.g., beta blockers, antidepressants, antiepileptic drugs).
  • Medications to reduce dizziness (e.g., antihistamines, vestibular suppressants).

3. Vestibular Rehabilitation

  • Specialized exercises to improve balance and reduce dizziness.

Common Triggers for Vestibular Migraine

  • Stress
  • Hormonal changes (e.g., menstrual cycle)
  • Sleep deprivation
  • Hunger or irregular eating patterns
  • Foods like caffeine, alcohol, chocolate, and processed foods
  • Bright lights or strong odors
  • Weather changes

Is Vestibular Migraine a Permanent Condition?

Vestibular migraine varies between individuals. For some, symptoms can be fully controlled with treatment and lifestyle changes. For others, symptoms may recur intermittently over time.

Impact of Vestibular Migraine on Quality of Life

If left unmanaged, vestibular migraine can significantly affect a person’s social, professional, and daily life. However, with proper treatment, these effects can be greatly minimized.

How to Cope with Vestibular Migraine

  • Identify and avoid triggers.
  • Adopt regular exercise and a healthy diet.
  • Practice relaxation techniques to manage emotional stress.
  • Consult a neurologist or ENT specialist regularly for ongoing management.
