Vestibular Clinical Assessment

Vestibular clinical assessment consists of a series of tests and examinations used to understand and diagnose issues related to a patient’s balance system. This evaluation focuses on examining the structures and functions associated with balance, such as the inner ear, brainstem, and cerebellum. The vestibular clinical assessment process typically involves the following steps:

1. Patient History Evaluation

  • Symptom Description: Detailed questioning of symptoms such as dizziness, imbalance, vertigo, tinnitus, and hearing loss.
  • Symptom Onset and Duration: Examining when symptoms began, their duration, triggers, and severity.
  • Medication and Medical History: Evaluating the patient’s current and past medications, chronic illnesses, past infections, or trauma.

2. Fiziksel Muayene:

Evaluation of Eye Movements: Assessing for nystagmus (involuntary eye movements), which may indicate a disorder in the central or peripheral vestibular system.

Head Movement Tests: Observing eye movements during head rotations in various directions. These tests help identify asymmetries in the vestibular system.

Dix-Hallpike Maneuver: A test performed when positional vertigo (e.g., benign paroxysmal positional vertigo – BPPV) is suspected. It involves positioning the patient’s head in a specific way to check for vertigo and nystagmus.

3. Vestibular Tests

VNG (Videonystagmography): Evaluates balance functions of the inner ear and central nervous system by tracking eye movements.

VEMP (Vestibular Evoked Myogenic Potentials): Measures the contraction response of the sternocleidomastoid muscle to sound stimuli, assessing the saccule (a part of the inner ear) and vestibulospinal reflex pathways.

Caloric Test: Evaluates how each ear’s vestibular system functions by applying warm and cold water or air to the inner ear.

Posturography: Measures how well balance is maintained in static and dynamic situations. The patient stands on various surfaces to assess the functioning of the balance system.

4. Hearing Tests

Audiometry: Evaluates hearing levels and detects hearing loss that may be associated with vestibular problems.

Tympanometry: Measures middle ear pressure, assessing the mobility of the tympanic membrane and middle ear function.

5. Imaging Techniques

MRI (Magnetic Resonance Imaging) or CT (Computed Tomography):
Used to examine structures such as the inner ear, brainstem, and cerebellum in detail.

6. Treatment Plan Development

Rehabilitation Programs: Vestibular rehabilitation includes physical therapies and exercises designed to manage balance problems.

Medication Therapy: Medications may be prescribed to alleviate symptoms or treat the underlying cause.

Surgical Intervention: In severe cases, surgical procedures may be required.

Vestibular clinical assessment helps identify the root cause of an individual’s balance issues and enables the determination of appropriate treatment options.
